PRODEDURE FOR MAKING APPLICATION FOR A
SPECIAL EDUCATION CONDITIONAL LICENSE

Application for a Conditional License can only be submitted to the State Department if a special education teaching position has been offered. There is a portion of the application that must be completed by the employing school district. This application is available at the State Department or the school district may have a supply. The Special Education Consultant at the State Department is Jane Heinsen. You may contact her at (515) 281-3437 or (jane.heinsen@ed.state.ia.us). A contact should be made if you have questions regarding your eligibility for this license.

If you have already been admitted to a special education program, you should meet with your faculty adviser to complete a plan of study. The plan should note which course work has been completed and should indicate when the remainder of the requirements will be completed. Usually a program guidesheet is used for this purpose. This completed plan, signed by the adviser, should be sent, along with your Conditional License application to Mike Cavin. A copy of this plan should also be sent to Lynn McMorris, N310 Lindquist Center, for inclusion in your graduate student file.

If you have not been admitted to a special education program, you should make application immediately. Once the application has been submitted, contact Lynn McMorris (319) 335-5358 or (lynn-mcmorris@uiowa.edu). You can also notify Lynn by including a note to her with the part of the application that is sent to the Office of Student Services in N310 LC. Request that she provide you with application notification letter addressed to the State Department. This letter and a blank copy of the program guidesheet should be sent with your Conditional License application to Mike Cavin.

When requesting this letter, be sure to specify the specific area of disability and grade level for which application is being made. As soon as admission has been finalized, make an appointment with your assigned adviser to design your plan of study. This must be done immediately in order to finalize your Conditional License. Please follow the same procedure for completing a plan of study explained in the above paragraph. Send the completed plan to Mike Cavin with a cover letter indicating that a Conditional License application is already on file at the State Department. Again, make sure a copy of this plan is sent to Lynn McMorris for inclusion in your graduate student file.

The decision to issue a two or three year Conditional License is made by Mike on the basis of the plan of study submitted. Questions regarding the issuance of the license should be directed to Mike. Questions regarding the plan of study procurement procedure can be directed to Lynn McMorris.
