INSTRUCTIONS FOR COMPLETING THE "APPLICATION FOR LICENSURE":
Obtain application materials by clicking on each item noted above. If you have difficulty downloading the materials, please call or stop by the Office of Teacher Education and Student Services in N310 Lindquist Center.
Section I to be completed by applicant
1. Supply the folder number appearing on your original Iowa License in blank
#1. If you don't remember or have
access to your number, you can leave it blank.
2. Provide a permanent address on your application. A new License will be mailed to that address directly from the State Department. It may take several weeks or months to receive your new license.
3. Application fees may be in the form of a money order or personal check made payable to the Board of Educational Examiners. Do not send cash. The State Department will not accept personal checks over three months old, so if you are in the process of completing licensure requirements, don't submit the application too early. If you are applying for your initial Professional Administrator License, the fee will be $85. There is no additional fee for the Evaluator Endorsement if submitted with your initial application for the Professional Administrator License. To add endorsements to your Professional Administrator License (example: Superintendent Endorsement) the cost is $50 per endorsement.
4. Sign and date the application.
Return all application materials to the address at the top of this
page.
5. Complete a program guide sheet for each approved administration program completed at The University of Iowa. The guide requires the signature of the adviser. Please obtain that signature before submitting application materials.
6. Upon receipt of your license, check over thoroughly. If changes are necessary, contact Lynn McMorris immediately. Changes made to your license after 30 days from issue date will result in additional fees.

The University of Iowa is your "Recommending
Institution" for all approved programs completed at this Institution.
Out-of-state licensure/certification applications frequently require a
recommendation from the "Recommending Institution" signed by the
Registrar, Dean, or Certification Official. These applications should be
sent to Lynn McMorris for processing. We will continue working with you on
an ongoing basis with all licensure/certification matters (outofst.html).
