INSTRUCTIONS FOR COMPLETING THE "APPLICATION FOR LICENSURE":
Obtain application materials by clicking on each item noted above. If you have difficulty downloading the materials, please call or stop by the Office of Teacher Education and Student Services in N310 Lindquist Center.
Section I to be completed by applicant
1. Supply the folder number appearing on your original Iowa License in blank
#1. If you don't remember or
have access to your number leave it blank.
2. Provide a permanent address on your application. A new License will be mailed to that address directly from the State Department. It may take several weeks or months to receive your new license.
3. Application fees may be in the form of a money order or personal check made payable to the Board of Educational Examiners. Do not send cash. The State Department will not accept personal checks over three months old, so if you are in the process of completing licensure requirements, don't submit the application too early. The fee for adding endorsements is $50 for each endorsement. If you are adding the K-6 and 7-12 endorsements for counseling, the cost will be $100.
4. Sign and date the application Return all application materials to the address at the top of this page. Endorsements requiring an experience verification are: reading specialist (verify teaching of reading specifically), special education consultant, director of special education and all administrative licensure.
5. Complete a program guide sheet for each approved program completed at The University of Iowa. If you have completed State minimum requirements no program guide is required, however, a copy of the analysis would be helpful. If the guide is not available on the web, please contact The Office of Teacher Education and Student Services in N310 LC.
6. Upon receipt of your license, check over thoroughly. If changes are necessary, contact Lynn McMorris immediately. Changes made to your license after 30 days from issue date will result in additional fees.

The University of Iowa is your "Recommending Institution" for all approved programs completed at this Institution. Out-of-state licensure/certification applications frequently require a recommendation from the "Recommending Institution" signed by the Registrar, Dean, or Certification Official. These applications should be sent to Lynn McMorris for processing. We will continue working with you on an ongoing basis with all licensure/certification matters (outofst.html). Institutional recommendation is not provided for any minimum standards endorsements.
