Application for Student Teaching

1. Complete the web-based personal information 
2. Complete the web-based student teaching application form 

   Be prepared to provide placement preference

3. Complete the web-based student teaching biographical data and professional information form

   Be prepared to provide a listing of the courses you will have completed in your program area and your specialization

4. (Secondary students including art and music) download and complete appropriate program guide sheet
   1. Select the guide sheet that represents the teacher education program that you will have completed.  Print a hard copy of this form and:
      1. Mark all of the courses that you will have completed by the time you have finished your student teaching
      2. Indicated the semester you plan to take course requirements for which you are not yet registered; and
      3. Indicate the institution where you completed transfer courses that are being used to fulfill program requirements
   2. Contact your education advisor prior to submission for an appointment to review and sign your completed program guide sheet.  Be prepared to provide your advisor with a current ISIS "degree evaluation"
   3. Submit the signed, completed program guide sheet to the Office of Teacher Education and Student Services, N310 Lindquist Center
   4. If you are considering graduation prior to student teaching -- and are receiving financial aid -- please be aware that you may become ineligible for certain types of financial aid once you have a bachelor's degree.  Please contact a Financial Aid Counselor in the Office of Student Financial Aid, 208 Calvin Hall, 319/335-1450.
5. (Optional) Complete the application for special site placement

   Be sure to include a current ISIS grade report print out as directed on the top of the application
