You will need Adobe Acrobat Reader software to download, read, and print the informational materials and application forms.  Free Adobe software is available for both Windows and Macintosh computers by clicking the "Get Adobe Reader" button.

*If you would like to receive a copy of the application packet by mail, please send a written request, along with your mailing address and payment of $25, to the REACH Office.  Upon receipt of your request, a packet will be mailed within 3 business days.
