Application Procedures

Update:  Now accepting application for Fall 2010!

Applications are currently being accepted for enrollment in Fall 2009. New enrollments are only accepted for the fall term of each school year. Applications will not be considered unless ALL required documentation is present at the time of review. There are 7 documents to print.

1. APPLICATION CHECKLIST PDF

d. ReferencesPDF—to be completed by one educator and other

   non-relatives who have know the student for at least six months

2. APPLICATION SUPPLEMENTS :

Test Administrator Instructions & Assessment FormsPDF

a. CREDENTIALS OF EVALUATORS (REQUIRED): The test administrator’s personal title and license or credentials on a separate page, name, address and phone.

b. ACHIEVEMENT TESTING (REQUIRED): An educational evaluation completed within the past 18 months with specific subtest scores and grade equivalents labeled from the Wechsler Individual Achievement Test- Second Edition (WIAT-II).  No written report is necessary.  Please use the WIAT-II Assessment Protocol sheet for reporting these scores.

c. INTELLECTUAL ASSESSMENT AND PSYCHOLOGICAL NARRATIVE (REQUIRED): An intellectual assessment completed within the past 18 months using the Wechsler Adult Intelligence Scale-III. Please include the complete narrative report, protocol and label (all subtest raw and scaled scores) and Verbal, Performance Full scale scores.  Please use the Intellectual Assessment Protocol sheet for reporting these scores.

d. SPECIFIC TESTING FOR LANGUAGE/COMMUNICATION CHALLENGES(OPTIONAL): If the student has significant language challenges (expressive or receptive), you may include an additional cognitive evaluation utilizing the Wechsler Nonverbal Scale of Ability (WNV), 2006 Edition.  A written report is preferred but not necessary.  Please list and identify all subtests, raw scores, T-scores and total scores.

e. INTERVIEW OF STUDENTS (REQUIRED): PDF A student interview conducted by the test administrator or psychologist. The interview should include the following questions answered by the student and recorded as verbatim stated and typed.


1. Give three reasons why you want to go to the University of Iowa REACH Program?

2. In the past 2 years, what did you do that you are most proud of accomplishing?


3. What would you like to be doing 5 years from now?


f. WRITING SAMPLE (REQUIRED):An original sample of the student’s writing should be completed during the assessment session. The writing should be in response to the question: write two things about yourself that you would tell to a new friend. Please use lined paper.

g. OTHER ASSESSMENTS (OPTIONAL): Copies of any psychological or educational tests and reports completed within the last three years and related school/professional reports are requested.

 

3. SCHOLARSHIP APPLICATION (OPTIONAL):PDF

a. Student Scholarship Application must be completed and submitted by February 15, 2009. Specific instructions for completing the FAFSA form for the REACH Program are included as a second page in the application document.


b. A Free Application for Federal Student Aid (FAFSA) will need to be completed and submitted to The University of Iowa prior to FEBRUARY 15, 2009. An electronic form can be obtained at www.fafsa.ed.gov.


Note: REACH requires an earlier deadline than FAFSA.


When completed, please send general and supplemental application materials, the scholarship application, and a $40 non-refundable application fee (check or money order made payable to REACH) to the following address:

A personal interview is required by the REACH Program.  An interview offer may be extended to an applicant and their family upon reivew of the completed admissions file by the Admissions Committee. You will be contacted regarding this phase of admissions.

Notification letters will be mailed approximately one week after a decision is made.

 

Please note that REACH will not accept forms that are submitted electronically, i.e., via e-mail.  All applications must be submitted in hardcopy and either mailed or hand-delivered.  Faxing applications and supplemental materials is discouraged.

If you would like to receive a copy of the application packet by mail, please send a written request, along with your mailing address and payment of $25 to the REACH Office.  Upon receipt of your request, a packet will be mailed within 3 business days.
