There are two forms that a student must complete before joining HOP and taking the Honors Seminars.

- HOP General Application

This is the form you must complete in order to be accepted into the
Honors Opportunity Program. Print and fill it out, then bring it
in to 600 Blank Honors Center to turn it in.
- Project Contract

After entering HOP you must complete the Project Contract in triplicate
(one for you, one for your mentoring professor, and one for the
Belin-Blank Center).

for Graduation with Honors

This form must be completed in addition to your Application for Degree
if you plan to graduate with honors in the College of Liberal Arts
or the College of Education. If you will graduate with honors in
two or more majors, please submit a separate form for each major.
- David Connie Belin Honors Award Nomination
Form

This award will honor graduating seniors who have completed all requirements
for the Honors Opportunity Program, culminating in a University
of Iowa degree with honors. Recipients will receive recognition
at the Spring College of Education awards ceremony. The David & Connie
Belin Honors Award carries a stipend of $500 per recipient.
