Special Handling of Credentials:

You can request in the box below that only selected references (list names) be included in a set of credentials or you can instruct the Educational Placement Office to hold the request until a specific reference is received. Unless you provide other instructions, copies of all references and your Iowa transcript (if you have provided it) will be included in your credentials.

Request by Fax:

If you need to have your credentials faxed to an employer, submit the employer's fax number and contact information in the box below.

Please Note:

When you submit this form, you will receive a confirming message to the Email address provided. The message will include an exact copy of what you have transmitted to the Educational Placement Office.

Upon receipt of this request, the Educational Placement Office will process your credentials promptly.
